What is a Primary Care Network?
Primary Care Networks (PCNs) are a made-in-Alberta approach to improve and better coordinate patient access to primary health care. Primary health care is the first point of contact most people have with the health system. In each PCN, a group of family doctors works with Alberta Health Services to deliver quality services to patients. A PCN is not necessarily a bricks-and-mortar building. Rather, it is a network of doctors and other health providers such as nurses, dietitians and pharmacists, working together to provide excellent health care. Primary care is the first point of contact most people have with the health system. It can be provided by family physicians and other health professionals, including nurses, dietitians, mental health professionals, pharmacists and therapists.
Family doctors and Primary Care Networks
The Calgary Foothills Primary Care Network is a group of more than 500 family doctors who work with teams of other health professionals to offer patients the best in primary health care. Our PCN serves more than 400,000 patients in north Calgary and Cochrane. Each Calgary Foothills PCN member physician’s clinic is a Health Home. It is a place where your everyday health needs can be met and your doctor will connect you to the help you need, when you need it. We work to achieve better health outcomes for our patients. We believe we can make a difference. Patients of Calgary Foothills PCN member doctors enjoy the comfort of having a family doctor and the health benefits that visiting a doctor regularly can bring. Calgary Foothills PCN is committed to making our employees’ lives at work rewarding and enjoyable. Find out more about working with the PCN. We look forward to hearing from you.
Wing Kei is a not-for-profit community-based organization that provides quality cultural-appropriate care to the seniors. “Wing Kei” means “glorify God” in Chinese. We are known for providing compassionate and loving care in a team-oriented environment. The Wing Kei story began in 1996 with a group of devoted Christians whose dream was to provide compassionate care for Chinese seniors. The first step was the inception of Wing Kei Villa, a culturally specific unit within the Bethany Care Centre. Under their auspices, a 30 bed long term care unit was opened to Chinese seniors. The Wing Kei Villa was an immediate success. In June 2001, the Federal Government, the Alberta Government and Wing Kei signed a commitment letter stating that each party would provide $4.5 million to fund the construction of the new Wing Kei Care Centre. Fundraising campaigns were carried out vigorously by Wing Kei supporters within the community from June 2001.
We know that when we succeed in achieving this goal, the whole community benefits.
Our approach to care is grounded in the aging-in-place model which recognizes that a continuum of care is required to meet the health needs of the population, seniors in particular. Our model emphasizes the value of keeping seniors as independent as possible – through their ability to be mobile, make their own decisions whenever appropriate and interact with their loved ones regularly. We support seniors to be as healthy as they can. We work as a team – caregivers, residents, families, support staff, physicians, health care professionals and volunteers – to respond to concerns through creativity and resourcefulness. Our interdisciplinary care team includes Licensed Practical Nurses, Health Care Aides, Registered Nurses, Physical Therapists, Occupational Therapists, Recreational Therapists, Therapeutic Assistants, Dietitian and Social Workers.
